This policy describes how Team Wellness LLC uses information collected on teamwellness.info. It is not a substitute for our HIPAA Notice of Privacy Practices, which we provide to patients.
Information we collect
If you submit the website intake form, we collect the name, phone number, email, service interest, and message you provide so we can respond and schedule care. If you book online, our scheduling tool processes your appointment information in order to hold the visit.
How we use it
We use inquiry information to return your call or email, prepare for a visit, and operate the clinic. We do not sell your information.
Sharing
Form submissions are stored by our form processor so the clinic can retrieve them. We may share information with service providers who help us operate the website or clinic, and when required by law.
